If You Have a Problem...（困ったときは）
If You Have Lost Your Passport （パスポートの紛失）
1. Submit a Notification of Lost Property (遺失届) at a Police Station.
2. Make a note of the receipt number of the Notification of Lost Property.
3. Contact the embassy or consulate of your country in Japan, and apply for reissuance.
If You Have Lost Your Residence Card（在留カードの紛失）
★ You should complete the formalities within 14 days of realizing that you have lost the card.
1. Submit a Notification of Lost Property at a Police Station.
2. Make a note of the receipt number of the Notification of Lost Property.
3. Complete the formalities for reissuance at the Hiroshima Immigration Services Agency.
※ Please take your passport with you.
If You Have Lost Your My Number Card（マイナンバーカードの紛失）
1. Contact the Personal Number Card Call Center, and apply to have the use of the card temporarily suspended.

2. Submit a Notification of Lost Property at a Police Station.
3. Make a note of the receipt number of the Notification of Lost Property.
4. Apply at the Citizens’ Affairs Division in the City Office to have the My Number card canceled and reissued. You will have to pay a fee for this.
