'Notification of Commencement of Usage' Postcard（使用開始届）

・The 'Notification of Commencement of Usage' Postcard will be hung on the door handle of the door to your apartment, or be left in the letter box on your door, in your letter box outside your apartment, or by the sink in your kitchen. If you cannot find the postcard, please check with your landlord.

・Please submit the 'Notification of Commencement of Usage' Postcard either before you start to use the service, or as soon as possible after you begin using the service. You can begin to use the water supply  right away. You cannot apply to commence use of the water supply by phone.

・Please fill in the application postcard referring to the example below. When you have filled in all the necessary details, cover the information with the privacy sticker, cut along the dotted line (キリトリ線), and put the postcard in a postbox. You do not need a stamp.

・If you wish to pay your water charges by bank account transfer, please complete the bank account transfer application at the bottom of the application postcard. You do not need to go to your bank, etc., to complete the formalities. The receipt for the bank transfer will be shown on the Notice of Water Meter Reading (水道メーター検針のお知らせ) that you will receive.

・If you do not complete the bank account transfer application, we will send payment slips. Please pay your water charges at a financial institution or convenience store, using these slips.

Household use: water supply used exclusively for daily life.

Business use: water supply used for all uses apart from household use and industrial use, or temporary use for building work, etc.

Industrial use: water supply used for manufacture or processing, where the diameter of the water meter is 40 millimeters or greater, or where a water storage tank is installed.

※ If the purpose for which the water supply is used changes, please inform the Water and Sewage Customer Center.

7、Only if you have moved house within Higashihiroshima City, please give your previous address.

8、If you wish to pay water charges by bank account transfer:

・Please fill in the bank name, branch name, type of account, account number, and the name of the account holder, as shown in your bank book. Water charges will be transferred from your account on the 26th day of odd-numbered months (if the 26th is a Saturday, Sunday or national holiday, the charges will be transferred on the next working day).

・If you paid your water charges at your previous address (within Higashihiroshima City) by bank transfer, and you wish to continue to use the same bank account to pay your water charges, please give the name of the financial institution and the account number, etc., and then circle ‘Continuation’ (継続).

9、Please stamp the form with the same seal as you used when setting up the bank account. If you do not have a seal, please sign the form with the signature you used when setting up the bank account.
