The Lump-sum Payment for Childbirth and Childcare(出産育児一時金)
When a person enrolled in the National Health Insurance Scheme gives birth, a Lump-sum Payment for Childbirth and Childcare is paid.
(Those who are enrolled in other health insurance should inquire with the health insurance they are enrolled in.)
Eligibility for application
When a person enrolled in the Higashihiroshima City National Health Insurance Scheme (an insured person) gives birth, the payment is made to the head of the household.
- As a general rule, the lump-sump payment is paid from the health insurance you are enrolled in at the time of giving birth, so if you are enrolled in another health insurance at the time of giving birth, please consult your medical institution or the National Health Insurance and Pension Division in the City Office. (However, if an insured person who has been enrolled in a company's health insurance for more than one year gives birth within six months of withdrawing from that health insurance, the lump-sum payment can be paid from the previous health insurance.)
- If the pregnancy has been 85 days (12 weeks) or more, the lump-sum payment will be paid even if the pregnancy is a stillbirth or a miscarriage.
- Please note that eligibility to receive the lump-sum payment expires two years from the day after you gave birth, and you will no longer be able to apply at that point.
Payment amount
500,000 yen per child (420,000 yen for births up to March 31, 2023)
If you give birth overseas:
404,000 yen for births up to December 31, 2021
408,000 yen for births from January 1, 2022 up to March 31, 2023
488,000 yen for births from April 1, 2023
Method of payment
There are the following methods for paying the Lump-sum Payment for Childbirth and Childcare:
- The direct payment system
- The recipient proxy system
- The head of the household applies for and receives the money after the birth
If the payment is made to an account other than that of the head of the household,, the head of the household must sign the application form.
The Direct Payment System(直接支払制度)
This is a system in which the Lump-sum Payment for Childbirth and Childcare from Higashihiroshima City National Health Insurance is paid directly to the medical institution. By having the Lump-sum Payment for Childbirth and Childcare made directly to the medical institution, the payment at the medical institution's counter is the amount minus the Lump-sum Payment for Childbirth and Childcare, eliminating the need to prepare a lump-sum for the costs of childbirth in advance.
How to apply
There is no need to apply in advance to Higashihiroshima City. When you are hospitalized, present your Health Insurance Certificate at the medical institution's counter and ask about using the direct payment system.
However, if the maternity expenses are less than the Lump-sum Payment for Childbirth and Childcare, you can apply to Higashihiroshima City after the birth and the difference will be paid.

The recipient proxy system(受取代理制度)
Prior approval from the hospital and an application to Higashihiroshima City are required. Applications are accepted from two months before the expected date of delivery.
(As of November 2024, there are no medical institutions in Higashihiroshima City that offer the recipient proxy system.)

Please have the medical institution fill out the recipient's section before submitting the form. - Documents that can prove birth, such as the Mother and Child Health Handbook (in the case of stillbirth or miscarriage, a doctor's certificate)
- Health iInsurance Certificate
- Bankbook in the name of the head of the household
- Identification document of the person submitting the notice (Residence card, My Number card, etc.)
The head of the household applies for and receives the money after the birth(出産後に世帯主が受け取る場合)
If you do not (cannot) use the direct payment system or the recipient proxy system, or if you give birth overseas, you will first pay the full amount of the birth expenses to the medical institution, etc. yourself, and then apply to Higashihiroshima City for the Lump-sum Payment for Childbirth and Childcare.
How to apply
If you give birth overseas, please apply after the person who gave birth returns to Japan.
