(※1) ‘Up to high school age’ means ’up to the last day of March in the year in which the child turns 18 years old.’

Those who need to apply（申請が必要な方）
Use the following flowchart to check whether you need to apply based on the situation as of October 1, 2024.
*1 ‘Children between 16 and 18 years old at the end of the 2024-25 financial year’: Children born between April 2, 2006 and April 1, 2009
*2 ‘Children up to 18 years old at the end of the 2024-25 financial year’: Children born after April 2, 2006
*3 ‘Children between 19 and 22 years old at the end of the 2024-25 financial year’ * Children born between April 2, 2002 and April 1, 2006
*4 Multiple child allowance: If there are three or more children up to 22 years old and one or more children up to 18 years old at the end of the 2024-25 financial year, allowances for the third child and onwards will be added.
The following documents are also required when applying:
1. Certification Request form
- Applicant's Health Insurance Certificate
- Identification document of the person submitting the notification (driver's license, My Number card, etc.)
- Copies of the applicant's and spouse's passports (pages with photographs and pages with last entry and exit records)
(Only if the person in question was living outside of Japan as of January 1, 2024)
2. Request for Amount Revision Certification
- Applicant's Health Insurance Certificate
- Identification document of the person submitting the notification (driver's license, My Number card, etc.)
3. Application for Separate Custody and 4. Confirmation of Custody and Living Expenses
- Identification document of the person submitting the notification (driver's license, My Number card, etc.)
*Other documents may be required depending on the situation.
Application period（申請期間）
Monday, September 2nd, 2024 to Monday, September 30th, 2024
*However, even after the above period has passed, applications can be made up until Monday, March 31st, 2025.

All applications except for ‘4. Confirmation of Custody and Living Expenses’ can be submitted through the ‘Pittari Service’ (My Number Portal).
