How to have your child vaccinated(予防接種の受け方)
- Prepare a record of any vaccinations previously received overseas (or elsewhere).
- Among the vaccinations your child has not yet received, check which vaccines are available in Japan, as well as the required number of doses and timing.
(Please refer to ‘Routine Vaccinations in Japan’ above.)
※ If you are unsure about your child’s vaccinations, bring any documents showing your child’s vaccination history to the Medical Treatment and Health Division. The staff will review your records with you and advise you on which vaccinations can be received.
- Obtain the necessary vaccination screening form (pre-vaccination questionnaire form) from the Medical Treatment and Health Division (City Office, Main Building 2nd Floor, Counter No. 8).
- Contact a medical institution in advance and visit the hospital/clinic with the required items.
