System of Subsidies for Medical Expenses for Children(乳幼児医療)

Overview(概要)

This is a system that subsidizes part of the medical expenses for infants, etc., when they receive medical treatment covered by insurance at a medical institution. The system of subsidies for medical expenses for infants and children in Higashihiroshima City has income restrictions.

Eligibility(対象児童)

The age range of those eligible for subsidies for medical expenses has been expanded from August 1st, 2023.

Children up to the third grade of high school who live in Higashihiroshima City and whose guardian's income is below the maximum amount. However, children who are already receiving other medical care systems (welfare, single-parent medical care, medical care for the severely mentally or physically handicapped, etc.) are not eligible for this subsidy.

Eligible children from age 0 to the third year of junior high school are eligible for medical expenses for both outpatient hospital visits and admission to hospital. In addition, eligible children from the first year of high school to the third year of high school (those whose birthday is between April 1 of the year in which they turn 16 and March 31 of the year in which they turn 18) are eligible to receive medical expenses for admission to hospital.

The Eligible Amount for Partial Subsidy(一部支給の対象となる額)

The amount obtained by deducting the amount of partial payment (individual payment by the guardian) from the amount paid by the individual for medical treatment covered by insurance (usually 20-30%).

If the parent/guardian is eligible to receive the subsidies, the following table shows the amounts to be paid by the parent/guardian when a child goes to hospital as an outpatient, or is admitted to hospital.

Outpatient

 

500 yen per day per medical institution, up to four days a month.

※ If a child goes to hospital as an outpatient five days or more in a month, from the fifth day the parent/guardian is not required to pay any amount.

※ There is no person contribution to be paid by the parent/guardian for medicine dispensed at a pharmacy where health insurance is valid, nor for assistive devices (glasses, etc.).

Admission

 

500 yen per day per medical institution, up to 14 days a month.

If a child is admitted to hospital for 15 or more days in a month, from the 15th day the parent/guardian is not required to pay any amount.

 Payments of subsidies are only made for items/treatment covered by insurance.

※ If you go to a medical facility with 200 or more beds without a letter of introduction, costs for services not covered by health insurance, such as the fee for the first examination, fees for health checks, costs of vaccinations, costs of orthodontic treatments, charges for higher grade rooms, charges for diapers, etc., and costs for dietary treatments while in hospital will not be paid.

How to Åpply(申請方法)

Application Counter

Children and Family Support Division (Kodomo Katei-ka), or a branch office or sub-branch office of the City Office.

If it is difficult to visit while the counters are open, applications can be accepted by post too. Please enquire for details.

Documents Needed For the Application

  • The Health Insurance Certificate of the eligible child;
  • If the applicant is currently living outside the city, a document showing the personal number (My Number) for the applicant (a My Number Card, a My Number notification card, etc.)
  • Proof of identity for the applicant (a My Number Card, a driving license, etc.)

(1)In the case of Giving Birth or Moving Into the City (for those aged between 0 and the third grade of Junior High School)

※ Even if you do not have all the necessary documents to hand, please apply within 14 days of giving birth or moving into Higashihiroshima City. (The application will be stamped as having been received. You can submit the necessary documents at a later date.)

If you apply after 14 days have passed, please be aware that any eligibility to receive subsidies will date from the date of application.

(2)In the case of Admission into Hospital (for those aged between the first and third grades of High School)

Please apply when your child is due to be admitted into hospital. The Certificate of Eligibility is not issued in advance.

When making the application, please give the hospital into which the child is to be admitted, and the length of the stay.

Income Limits(所得制限について)

There are income restrictions in the judgment process for eligibility.
*In some cases, such as in the case of a new application due to moving into a new city, the income year required for screening may be in the past, depending on the child's date of birth. Please contact us if you have any questions.

※Income subject to assessment is as follows:
The amount of gross income minus necessary expenses (or ‘amount after deduction of employment income’ on the tax withholding form for those earning only employment income) is called income.
In addition, the following items are deducted from income, respectively.

  • A flat amount of 80,000 yen as deduction for social insurance premiums
  • Deductions for medical expenses and deduction for small-scale enterprise mutual aid premiums, etc., if any
  • A set amount if the individual and his/her dependents are receiving deductions for disability, etc.

* Please use these notes as a general guideline to check your eligibility.

A Notice of Rejection of Eligibility will be sent to those who are not eligible to receive a Certificate of Eligibility because they were over the income limit. This rejection of eligibility is for the period of eligibility until the end of the month following the child's next birthday.

However, eligibility from the month following the month of the child's next birthday will be reviewed based on the new year's income, and may result in eligibility due to changes in the amount of income or number of dependents.

If you receive a Notice of Rejection of Eligibility, please re-apply in the month following your child's next birthday, along with the required documents.

Revisions from the 2008 Financial Year

Due to the enforcement of the Cabinet Order for Partial Revision of the Order for Enforcement of the Child Allowance Law, etc., the following items were revised from June 1, 2008.

(Application of Special Deduction for Long-term Conveyance Income and Short-term Conveyance Income)

In cases where special deductions for long-term or short-term transfer income prescribed in the Act on Special Measures Concerning Taxation are applicable, the amount of such deductions shall be deducted from the amount of long-term transfer income and short-term transfer income.

Deemed application of the widow(er)'s (widower's) credit *Repealed from the income taxation for the 2021 financial year.

Among unmarried single parents to whom the widow(er)'s or widower's exemption under the Local Tax Law does not apply, those who meet the requirements shall be deducted in the same manner as the widow(er)'s exemption under the Local Tax Law.

Other Times When Paperwork Needs To Be Completed(その他、手続きが必要なとき)

  • If your address changes (moving within the city, moving out of the city);
  • If your Health Insurance Certificate changes;
  • If you lose your Certificate of Eligibility;
  • If the person who is the center of the household’s finances changes;
  • If your name changes, etc.

Renewal of Certificates of Eligibility (only for children aged 0 to junior high school 3rd grade (15 years old)(受給者証の更新)

The Certificate of Eligibility is changed every year on the first day of the month following the month of the child's birth (or the current month for children born on the 1st of the month), and the child’s eligibility will be judged based on the income of his/her parent(s)/guardian(s) for the new financial year. In principle, it is not necessary to apply for renewal. However, you may be asked to submit the necessary documents if the required items cannot be verified.
In addition, if you receive a Notice of Rejection of Eligibility, you will be required to apply again. If you wish to have your child's eligibility reviewed, please re-apply in the month of the child's next age birthday, along with the required documents.
If the income requirements are met, a Certificate of Eligibility will be issued for the child in question.

The new Certificate of Eligibility will be sent by the expiration date of the old one.

Claim for Refund of Medical Expenses(医療費の返還請求)       

If you have incurred medical expenses covered by insurance that exceeded your contribution during the valid period of your Certificate of Eligibility, you may request a refund of the medical expenses.
